DrDoctor is the system Care ADHD uses to send you form assessments to complete. We do not manage your care. You can find further details on this page: Centre for ADHD Research and Excellence
How to upload your Summary Care Record or ID verification
Once in the Form page, click on "Choose Files" and upload the document. Files must be in JPG or PNG format and under 20 MB. If you've taken a photo on your phone, it's most likely already in one of these formats.
If you're having trouble saving your file in a supported format, please contact Care ADHD directly — they can help you find an alternative way to share it. You can reach them via their website contact form or by calling 020 464 27098 for adults and 020 464 27103 for U18s.
Do not share your documents with the DrDoctor support team, we can't help upload them for you.
Long assessment forms, for example DIVA-5
Your progress saves automatically, so you can leave and come back at any time. To review what you've entered so far, log in to your patient portal at https://drdoctor.thirdparty.nhs.uk.
When you're ready to submit, make sure every field is completed and within the required character limits because the form won't submit if anything is missing or over the limit.
I've completed my forms and I have a follow up question.
DrDoctor doesn't have visibility into form submissions or appointment scheduling. Once your forms are submitted they will be reviewed by the Care ADHD team and they will get in touch in due course. If you have any questions about the next steps, you can contact them directly via their website contact form or by calling by calling 020 464 27098 for adults and 020 464 27103 for U18s.
I've been asked to complete the same form twice.
There are two common reasons this can happen:
- Routine re-issue — Care ADHD may send the same form again after a certain amount of time has passed.
- Accidental re-trigger — If this happened straight after submitting, you may have clicked the original invitation link again, which can reassign the form. To avoid this, always log in directly at https://drdoctor.thirdparty.nhs.uk rather than using the link in your SMS or email.
My details aren't being recognised
When you log in, your details are checked against the records held by your care provider. DrDoctor doesn't hold or manage this data, so we're unable to make changes on your behalf.
If you're seeing an error with your surname, date of birth, or postcode, please contact Care ADHD directly so they can check and update their records. You can reach them via their website contact form or by calling by calling 020 464 27098 for adults and 020 464 27103 for U18s.
I can't find my form after logging in
If your form isn't showing after you log in, you may be registered with more than one care provider on the DrDoctor platform. In that case, the portal should prompt you to choose which provider's records to access but a previously saved session on your device can sometimes skip this step.
Try clearing your browsing history, or follow these steps:
- Go to https://drdoctor.thirdparty.nhs.uk
- Scroll down and click "Sign in with another care provider"
- Enter your last name, date of birth, and postcode, then click Continue
- When prompted, select Care ADHD from the list
- A security code will be sent to your saved phone number — enter it to access your assessments
If this still doesn't work, it may be that you saved login details do not match the data you entered. To solve this please see the advice here My details aren't being recognised.
